  1. How do I become an AFI member?
    Call the number above and ask for a customer service representative who would be happy to walk you through the process.
     
  2. Do I need a physical exam to be accepted by AFI?
    No physical exam is needed for any of our health insurance products.
     
  3. What is a Mini-Med? HMO? POS?
    A Mini-Med or Limited Medical plan is a health insurance plan with limited benefits. The benefits may not fully cover all of your medical services. You are allowed to choose any doctor you want and are reimbursed a specific amount for each service based upon the plan you have chosen.
    A HMO plan is a health insurance plan that offers prepaid comprehensive health coverage for both hospital and physician services. A HMO contracts with health care providers and members are required to use participating providers for all health services.
    A POS plan is health insurance plan with a dual option. It allows you to access a network (as with an HMO) and also allows you to go outside of the network where you are subject to a deductible (an amount the member must pay before an insurance company pays any benefit) and co-insurance (a shared amount that the member and the insurance company pays for benefits).
     
  4. How quickly is coverage in effect?
    Coverage with all AFI programs begin on the 1st of the month after enrollment
    Materials are received.
     
  5. Is it beneficial for a start-up business to offer self insurance through AFI?
    AFI’s staff of insurance professionals, all with over 10 years of experience are well equipped to walk the new business owner through all the pitfalls of starting a health insurance plan for a new business.

  7. Is the AFI membership fee per person, per family or per business?
    AFI membership is based upon either Single or Family membership. Membership is easy and painless. Please call one of our customer service representatives who will be happy to answer any and all of your questions.
